I understand that by signing this Consent Form I am giving Healthpilot Technologies LLC, its employees, and agents (collectively, “Healthpilot”) the right to use my name, testimonial, and image, likeness or an actor portrayal image, and video regarding Healthpilot's services in Healthpilot's marketing initiatives and materials. I understand that my name, testimonial or image (likeness or actor portrayal image) and video may be used in various marketing mediums of Healthpilot for purposes such as publicity, illustration, advertising, and web content. I authorize Healthpilot to amend, copyright, use and publish these materials in both print and electronic formats for purposes of publicizing Healthpilot without limitation. In addition, I waive any right to inspect or approve the finished product, including written copy, where my name or testimonial appears. I understand that my consent remains in effect until I revoke it. I may revoke this consent upon email notice to Healthpilot at [email protected]. I understand it will take Healthpilot a reasonable amount of time to act upon my revocation and that my revocation will not apply to marketing materials made available to the public prior to my revocation.
